Urinary blood that's can’t be seen without a microscope which is called microscopic hematuria and reported in urine test as the number of red blood cells per high power field (HPF),
In mild hematuria:
Slight change in color or no change in color of the urine as well as red blood cells appear under microscopic finding section of your urine report.
In gross hematuria,
The urine appears pinkish, red, or smoky brown, and there may be small blood clots.
Microscopic RBCs exist in variable count from 20 per HPF and up to over count fields.

  • If you are eating large amounts of certain coloring foods as beets, berries, or rhubarb.
  • It's from not drinking enough water which leads to heavily color orange urine in toilet, which means not enough water is being consumed. (only severe dehydration can lead to kidney failure)
  • Not letting out all your urine, holding your bladder,
  • Drinking too much soda's
  • If you empty your bladder after sexual intercourse.
  • You’re doing strenuous exercises, only urine color change and shouldn’t see any visible blood in urine which is due to dehydration during training or due to repeated jarring of the bladder and thus lead to blood in the urine. Blood in urine caused by exercise should goes away on its own during 48 hours.
  • You’re on some common medications as: some medications could provoke blood in the urine such as Antibiotics (such as Rifampin), analgesics (Aspirin), and anticoagulants (Warfarin, Phenytoin, and Quinine) which commonly known to cause blood in urine.

If nothing from the above, then it may one of the next.
Mild infection and small stones:
You may have a tiny urinary tract infection (UTI), that can be cleared up after a few days or with antibiotics if prolonged.
Sometimes they go away on their own, but if your doctor thinks it is a problem she will ask you to retest and may put you on antibiotics.
Could be too much calcium and as it passes through kidneys can irritate kidneys, found in urine test as crystals of calcium oxalates or other salts which precipitates in urine.
Moderate blood in urine
Kidney diseases at the early stages and renal infections may drop 10 – 40 cells per HPF with pus cells and casts also appear in urine analysis report.
Bladder infections, urethra infection, and prostate infection show moderate hematuria but with over 100 pus in severe cases, the blood in this case may due to blockage of urine, inflamed tissue on the walls of organ.
Also any severe UTI can cause moderate to marked hematuria with over count puss cells in turbid dark urine.
Reasons of heavy blood in urine of male:
What does blood in my urine mean? it's heavy for last 24 hours and I’m a male.
It's important to determine the reason for the bleeding, let’s start the prognosis journey:

  1. Injury to the upper or lower urinary tract:
  2. A car accident
  3. A bad falling onto your back
  4. Got hit in the balls so hard and you pissed blood, should not last for more than 24 hours.
  5. Blood in urine with testicle hurts, stinging feeling at the tip when peeing, and pinkish colored urine, it could be an injury, trauma, or infection of your genitals.
  6. Your penis hardly restricted in the tight underwear, after that you may see bloody urine for 24 hours less or more.

If all the above is not your case, then the blood may be due to a disease:
STDs can also be a reason of bloody urine which could be very frightening
Depending upon the type of infection, STDs that leads to genital lesions may be result in genital warts, painful blisters, or ulcers:

  • Chlamydia
  • Genital Warts
  • Gonorrhea
  • Hepatitis B virus
  • Herpes virus
  • HIV & AIDS
  • Human Papillomavirus (HPV)
  • Molluscum Contagiosum
  • Pelvic Inflammatory Disease (PID)
  • Pubic Lice or Crabs
  • Scabies
  • Syphilis (RPR test)
  • Trichomoniasis (Trich)

STDs that cause urethritis and symptoms similar to a urinary tract infection, including painful or burning sensation during urination and abnormal discharge from the urethra.
Peeing almost pure blood instead of urine in males after sex for over 4 days which is not painful
Sexual partner if had a sexual disease as Herpes virus and Chlamydia will transfer the infection to you, although excessive masturbation will rupture vesicular tubes in the scrotum (sac of man’s eggs) and cause bleeding which can lead to more serious problems. Getting your rod restricted while coitus or bent while masturbation may cause possible urinary bleeding for the next 48 hours less or more.
Urinalysis report should include:
Pus cells over count and many RBC’s and proteinuria is sexual infection is the case.
Marked RBC’s and few pus cells if the accidental events is the case.
Although few RBC’s and Few Pus in urine do not exclude STD infections.
Stones cause bloody urination in men
blood from gallstoneRenal calculi (kidney stone) or cystolith ( a name given for bladder stone)
The laboratory is frequently requested to perform a urinalysis when patients presenting with severe back and abdominal pain are suspected of having renal calculi. In such cases, hematuria is usually of a small to moderate degree, but its presence can be essential to the diagnosis.
If Pain in the flank, back, lower belly or groin is commonly associated with kidney stones
You may get a fair bit of blood in your urine from kidney stones with or without pain, although most people experience horrific pain, others not even know that they have calculi and pass stones that are small.
Usually stone starts with no pain and RBCs few to many in urine test, such signs are considered early markers of urinary tract stones. Pelvic Ultrasound is recommended.
Urinary tract blockage by kidney stones or large stones can result in NO to many RBCs in urine test and bleeding when peeing in bathroom. Usually the urethra blockage by a stone lead to moderate hematuria.
Benign which means it’s non-cancerous enlargement of the prostate:
Known as benign prostatic hypertrophy (BPH) is a common condition in older men that lead to blood in urine.
Finally, get to know:
The last thing you may concern about after all of that reasons is Cancers, Chronic diseases such as diabetes, hypertension, sickle cell anemia and viral infections which could also be the cause for blood in the urine.
But those are likely what causes blood in urine of male older than 40.
Do you want to know all causes of bloody urine?
 Recommendation:

  • Basically, the appearance of the urine is usually not a clue as to the cause, because some foods, medications, or doings can alter the urine color.
  • Usually doctor’s follow up by another repeating urinalysis after course of medication but they usually not relying much on the changing numbers of RBC’s.
  • Few red blood cells in urine test may have nothing to worry about.
  • Visible bleeding even with no pain is a serious matter and must be revised by a urologist.
  • I'd suggest, consulting with your personal doctor or urologist for more information.
  • You may call your doctor back and tell him your nervous about change in your urine, and remember the treatment depends on the underlying cause.
  • Take a blood test for general checkup every 6 months at least to avoid sudden diseases.

In the meantime, however,

  1. plan to drink up to 10 glasses of water for the day.
  2. Consume a lot of a very wide variety of fresh fruits and vegetables inclusive of raw vegetables and green leafy vegetables, drink plenty of vegetable juices.
  3. Many advices out there recommend eating seeds including pumpkin seeds Include celery, parsley, carrot, cabbage, eggs fish, kale, turnips, broccoli, garlic and onion in your daily meals.
  4. Cranberry juice is perfect in UTIs and hematuria but could mask symptoms, so that you may postpone it to after doctor visit.
  5. If you run for long periods of time on a hard surface such as concrete, try running on a track or something soft, as a field of grass instead, and don’t forget to change your running shoes every 6 months or so.
  6. Have your testicles swollen or lumpy? do you have any pain while urinating? These are things to check for and mention to the doctor.
