Why my blood tests show high blood glucose levels (hyperglycemia)?

It is a common question by any one went to the medical lab and tests a blood sample for his glucose and get lab results with elevated levels in blood sugar, other questions will be in mind may be:
Is high glucose level is caused by disease?
Am I in trouble get my lab test results high in glucose?

Blood glucose test is a common lab test and may be carried to patient at any time of the day and reported as random blood sugar levels in blood, if the RBG test shows high results it is recommended to carry out fasting blood sugar test (FBG) after six hours fasting, and glucose test after two hours postprandial test

My doctor said that my diabetes is of second type, what does this means?
First type diabetes mellitus is medical condition in which the highness of blood glucose level due to no insulin in the blood due to tumor of insulin cells in pancreas or high glucose levels due to insufficient amount of insulin enters blood stream in appropriate time, lab test results in this case should be higher than 500 mg/dl in patient‘s serum and (+++) in urinary glucose test by strep and ketones appear.
